We will be hosting free vaccination booster clinics (Moderna, Pfizer) Saturday, Jan. 22 and Wednesday, Jan. 26. If it’s been six months since your vaccination and you are 12 years of age or older, please sign up by clicking here. We will be hosting the clinics at the LEO Center (Room 112), 300 S. West Dr. in Leander. Because the link is the same for both clinics, please pay close attention to the exact clinic for which you are signing up.

Jan. 26 (LEO Room 109, 2–6 p.m.)
After scheduling your appointment please download and print your consent form. Please bring the consent form, your insurance cards, and CDC vaccination cards from your previous dose(s). If you do not have a vaccination card, a picture will suffice so that we can verify the timing and manufacturer of your last dose and the manufacturer of your last dose.
