LISD will hold its next COVID-19 vaccination clinic for 5-11 year-olds from 3:30–7:30 p.m. on Thursday, Nov. 18 at the LISD Technology Building (315 S W Dr, Leander). Appointments are required, as there are 400 doses of the first shot of the Pfizer vaccine available at this clinic.
Parents/Guardians will need to fill out and sign consent and release forms to have their child vaccinated. These forms are available at the clinic OR can be printed out at home as part of the appointment sign-up process. Please be sure to bring a copy of an insurance or Medicaid card if applicable. PLEASE NOTE: Even if a patient doesn’t have insurance, the vaccine is still free.
Parents or guardians are not required to be at the appointment for the immunization to occur, but the immunization cannot occur without a signed consent and release.
